Observations; A group of children ages 18 - 41 months were in one room grouped together as Toddler/twos. The grouping for these ages is not in compliance. Toddler ages are from 12-30 months and twos are from 24-35 months. 1240-04-03-.06 SUPERVISION. Corrective Action: After TA was given on grouping, the corrective action plan developed by the Director is that the group will be divided into two groups. The toddler group will be from 12-30 months at a ratio of 1:3 and the 2 1/2 -3 year room will range from 30 months - 47 months with a ratio of 1:4. The plan corrected the violation.

Observations: A staff file lacked a physical and had only 2 of 3 references. 1240-04-03-.05 OWNERSHIP, ORGANIZATION, AND ADMINISTRATION. Compliance Statement: The Director is planning a check list with the forms that must be in the staff files. She will double check the files. The Director will send proof to PE that the file is complete.
