Verification of correction was sent on 10/16/19. Unable to enter that date as the corrected date. 1240-04-01-.08 RECORD KEEPING. Corrective Action: A plan of corrective action was completed on site. The owner/director called the mother. The grandmother picked up the child. The owner/director noted that the child would not be allowed back in care until she had the immunization certificate as required by licensing rules.
