Educator stated that they forgot to count the children when returning from their activity and did not know that the child was missing until the director returned the child. 1240-04-01.11: SUPERVISION. Corrective Action: Agency will be training educator again on supervision procedures and will be getting additional training from the Child Care Resource and Referral Office.

Observation: Child was separated from his class after they lined up and went across the hall to the lunch room. He didn't follow them into the room and another teacher saw in him in the hall and him return to his classroom. She alerted the director and child was reunited with his class. It was estimated that he was not with his group for 1 to 3 minutes but the other educator was able to see him in the hall and go into his room at the door. 1240-04-01-.11 Supervision Corrective Action: Child was returned to his classroom. Educator loudly (so director can hear) counts when leaving the room and getting to the next room and when she returns to her classroom. She will also let director know if she needs something heated up in the lunchroom so someone can watch her classroom instead of taking the class with her to do that. Agency will be training educator again on supervision procedures and will be getting additional training from the Child Care Resource and Referral Office.
