Observation: During the agency’s 8/29/19 assessment, an infant was not touched at a 15 minute check. Instead, the educator just looked at the infant asleep in the crib without touching the child. All infants need to be touched every 15 minutes because of the possibility of Sudden Infant Death Syndrome (SIDS). 1240-04-01-.11 SUPERVISION Corrective Action: Program Evaluator gave technical assistance and a plan of corrective action was completed Child Care Resource and Referral was contacted; the infant/toddler specialist will come out to work with the infant room educators.

Observation: During an unannounced visit three classrooms were found to be out of the required adult:child ratio. The 3-5 year old classroom was 1:18, the required ratio is 1:13. The 13-24 months classroom was 1:8, the required ratio is 1:6. The 2 year old classroom was 1:9, the required ratio is 1:7. The ratios were corrected on site when a staff member arrived and was able to open the 2.5-3 year old classroom. 1240-04-01-.22 Specific Requirements for Child Care Centers. Corrective Action: The director reported that a staff member had called in that morning and she contacted a substitute. The violation was corrected on site when the substitute arrived. The director will be working on a place to ensure that the center is staffed adequately at all times.
