Licensing Consultant (LC) Clark conducted a safety plan monitoring visit for safety plans in place at the agency. During the visit, the individuals on the safety plans were present, and supervision logs were maintained. Agency understands that the safety plan is in place until a lift letter is received from DHS Licensing.
