Summary of [Enter Visit purpose] Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint invesigation If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. The department observed the bathroom the tub with two (2) mop buckets, a mop, and a shower curtain hanging that was accessible to the children during hours of operations.

Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation. Technical Assistance Provided: 1. TA provided is to ensure children under two are not watching television. 2. TA provided is to ensure all children are signed in by a parent/guardian. Ensure they have time in/out. 3. TA provided is to ensure children are never left alone. 4. TA provided is to ensure children UNDER 30 months are not combined with children OVER 30 months. 5. TA provided is classrooms can only be combined during the first/last hour of the day. 6. TA provided is to ensure educators inform parents of any scheduled media viewing. 7. TA provided the critical violations can be found on page 14. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. During an unannounced complaint visit, LC observed the educator leave children unattended for two to three minutes. During an unannounced complaint visit, LC observed five children not signed in with the name of the individual who brought the child. During an unannounced complaint visit, LC observed children watching television. LC asked if were parents informed about the children watching television and F. Transou stated "no". During an unannounced complaint visit, LC observed age grouping from 6 weeks to 12-year-olds in the same classroom. During an unannounced complaint visit, LC observed the educator leave children unattended for two to three minutes.
