The visit conducted today included monitoring the agency's compliance with Licensing Rules and regulations due to multiple complaints. One educator was found working with children before all of her pre-service trainings were completed. The educator was able to get all training requirements completed while LC was on site. 8 children were left unattended in the classroom for approximately two minutes.

The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and a complaint investigation. An incident occurred while the children were playing on the playground; one child picked up and threw an empty coffee can at another child. The coffee can is used as an exploration tool for children to fill and empty with mulch while playing. The child who was hit by the coffee can suffered a scratch on his head. The center did not provide an incident report to the parents in this case.

See Below for Detailed Information Observation: During an unannounced visit for the complaint investigation. LC Turner learned through interviewing the educators the Ladybugs classroom ages eleven (11) months to thirty (30) months frequently had a ratio of one (1) to six (6), when the ratio should have been one (1) to five (5). This would happen in the mornings from 8:30 to 9:00 am. 1240-04-01-.22 Specific for Child Care Centers Plan of Corrective Action: The center has hired another educator in that classroom to ensure the adult:child ratio is being maintained. Observation: During an unannounced visit for the compliant investigation. Licensing Consultant (LC) Kirsten Turner confirmed through interviews of the educators that during the week of August 9-13, 2021 a three (3) year old child was left unattended at the end of the hallway after outside play time for about three (3) to five (5) minutes. The child was found by another caregiver and returned to her classroom. The educator in the child’s class had not properly tracked the children and was unaware the child was missing. 1240-04-01-.11 SUPERVISION Plan of of Corrective Action: The educators have been given instruction on tracking the children before leaving and upon arrival in different areas in the facility. the educators are to count the children as well as take attendance to ensure that all children are accounted for before proceeding to a new location. Observation: During an unannounced visit for the complaint investigation. LC Turner observed on August 23, 2021 the bumble bee classroom with children ages one (1) to two (2) yrs. old had a ratio of 1:11. The ratio should have been 2:12. 1240-04-01-.22 Specific for Child Care Centers Plan of Corrective Action: All educators are aware of their classroom ratio requirement. If they need to leave the classroom they are to notify management so a floater or another educator may be in the room to maintain ratios.
