Summary of Complaint/ Serious Incident Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and a Complaint /Serious Incident visit. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. It was observed that an educator shamed a child and grabbed him by his arm to bring him into the classroom. The incident was not reported to DHS or DCS the same day. It was also not reported to the parent the same day.

Summary of Serious Injury/Incident Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations. Technical Assistance Provided: 1. Playground supervision 2. Face to Name Checks when transitioning. Violations were received and include: 1. Supervision- 1240-04-01-.11 2.Playground Supervision- 1240-04-01-.11 Correct Action Plan (CAP) completed. Observation: Child was left unattended on the playground. Observation: Roll call or Face to Name check was not done upon arrival in the classroom. Playground Supervision- Roll call before leaving classroom and upon arrival at playground and prior to leaving playground and upon arrival in classroom.
