During a safety plan monitoring visit on 5/15/2026, FS Pass observed the Lovely Lion, 3 year old class, transition to the bathroom. The educator did not do face to name roll call upon arriving at the bathroom. She asked the children to sit in the chairs that were located outside the bathroom and sent two children into the bathroom.

While FS Pass was in the hallway observing another class there was a 4 - 5 year old class left unattended for less than one minute. The educator left the class unattended to ask the office manager a question. The office was not in direct sight or sound of the children in the hallway. FS Pass observed the educator return and she took her class to the playground. She completed face to name ratio at the playground.

On 5/15/26, FS Pass was conducting a Safety Plan Monitoring visit. FS Pass was observing the three year old classroom transition to the bathroom. Once at the bathroom 3 children sat in blue chairs and two children went into the bathroom. The educator was not in direct sight of the children in the bathroom for about a minute. She then moved to a part of the hallway where she could see the children in the hall and the children in the bathroom.

The Owner and Management have the ultimate responsibility for the safety and well-being of children in care by ensuring that qualified staff members are hired to work for the agency, and that after hiring all staff members perform in compliance with child care licensing requirements.

On 4/10/2025, A three-year-old female child came out of the bathroom and was asked to sit down on the wall. The educator grabbed the child aggressively and sat her down on the wall. Then, a three-year-old male came out of the bathroom and was asked to sit on the wall. The three-year-old male did not sit down and was getting up. The educator grabbed the three-year-old male by the arm forcefully and caused him to hit his head on the wall. He was checked out, and he did not have any injuries or bruises. He has not complained of it hurting. Juan was terminated from the job on 4/10/2025. The children's parents will be notified after the DCS call and DHS.
