During the complaint investigation on November 10, 2025, LC Woods discovered that two school age children who were not enrolled were on site. The agencey is currently licensed to serve children 12 months- 5 years old. A complaint stated that on November 10, 2025, the director of the agency transported a 5 year old child to her residence. The director stated that she needed to go to her house because she left her cell phone at home and the agency phone was not working because it needed to be charged. She transported the 5 year old child to her home to get her cell phone and returned back to the agency. During a complaint investigation on November 10, 2025, LC Woods found that the agency's phone was not working due to the phone not being charged. The director stated she left the agency to retrieve her personal cell phone from home, as no other working phone was available. On November 10, 2025, during a complaint investigation, LC Woods determined that the agency was not open during its posted hours of operation and did not notify the LC. It was confirmed that the agency director had temporarily closed the facility to return to her residence in order to retrieve her personal cell phone. During a complaint investigation on December 9, 2025, LC Woods discovered that the director wrote her name on a child's application giving herself permission to release a child to herself.
