See Below for Detailed Information Observation: During an unannounced visit on 7/29/2021. LC observed two educators on the playground. One educator was seated on the playground and the other educator was standing less than 2 (two) feet away from the educator who was seated. They were facing each other and it appeared they were talking and not supervising the children. This was observed for approximately 2 – 3 minutes. This did not match the playground supervision plan for the agency. 1240-04-01-.11 Supervision Plan of Corrective Action: The playground supervision plan will be posted to all the doors that are leading out to the playground.

See Below for Detailed Information Observation: On February 10, 2021 it was confirmed as part of the complaint investigation from February 8, 2021 that a child in the 4-5 year-old class room was bathroom for discipline when throwing a temper tantrum. Bathrooms are not a safe or appropriate place for children to be alone when upset or for discipline. Violation is being cited today as the complaint is being closed. 1240-04-01-.15 Program, Language, and Literacy Development Corrective Action: All educators will get 3 hours of training, and all educators will attend a staff meeting focused on appropriate discipline and classroom management. All training will be completed my 3/19/21.

An unannounced visit was conducted in response to a report concerning a possible injury to a child that occurred during care. The report stated that a young child had complained of leg pain after naptime and was later diagnosed with a fractured tibia at a local emergency room. During the visit, Licensing staff reviewed internal documentation, including incident reports, educator statements, and classroom camera footage. Footage from July 25, 2025, showed a staff member directing a child to their mat during naptime preparation. In the video, the staff member is seen taking the child by the arm and turning swiftly, resulting in the child falling to the mat with some force. While the child appeared to recover quickly and showed no visible distress in the moment, a limp was later observed on camera as the child moved around the room.

Observation: During a complaint investigation conducted on August 10,2021 at the center. It was confirmed that the infant room ages 6wks to 12months, a three (3) month old was left unattended on a Boppy pillow while the caregiver changed diapers. During this time the infant fell asleep in the Boppy pillow. The educator reported when she noticed the child asleep the three (3) months old child was moved. The child should not have been left in a soft surface where the child could fall asleep and without an educator in direct supervision during floor play. 1240-04-01-.11 SUPERVISION. Plan of Corrective Action: All educators received supervision training. The infant educators also received coaching and development about floor play during diaper change time. Safe sleep procedures were also reviewed and children are also immediately transferred to the crib.

Observation: The Pre k Classroom had a ratio of one (1) to fifteen (15) children ages four (4) to seven (7) yrs. old. This was due to a (4) four-year-old being transitioned too soon. The grouping of the children should have been five (5) to twelve (12) years old. 1240-04-01.22 Specific Requirements for Child Care Centers Corrective Action: Agency will not transition until children are age appropriate for the class. Correct ratio and groupings will be maintained. Plan of Corrective Action: Upcoming Pre K students with no longer be transitioned while summer camp is in session or while schoolagers over the age of five are present.
