On 4/28/2026, the school conducted a monthly fire drill during rest time. The educators called face to name from memory; not an accurate roster. As a result, a three year old child was left unattended while she slept on her cot for approximately 7 minutes. After a lapse of time, the educators realized that the child was missing and notified the administration. Administration immediately went inside and located the child who was still asleep on her cot in the classroom.

Agency self-reported that a child was unattended on the larger playground (4 years old) for 3 minutes. Another educator from inside saw the child left outside alone and alerted other educators. Video foodtage is currently unavailabe due to digital storage issue but is being worked on so that PS can review.
