On 1/14/2026, LC Charlotte Spells conducted an unannounced visit to investigate an anonymous complaint. During the investigation and after interviewing each educator present, it was found that on 12/10/2025 director, saw an educator use inappropriate discipline on a child by tapping the child on their bottom area. Director Paula removed the educator from the classroom, and they were terminated the same day of the incident. The incident was documented for the provider's records but was not reported to DHS or DCS. The parent of the child involved was notified about the incident, but an incident report was not provided or signed by the parent. Because inappropriate discipline by an educator was not reported to DHS and DCS, the provider did not properly report suspected child abuse. Observation:On May 3, 2021 Licensing Consultant Kirsten Turner verified an incident of a three (3) year old child being left alone in a classroom that occurred on Friday, April 30, 2021 as reported by the director who was not on site at the time. On April 30, 2021 around 2:30 pm the two and a half (2 ½) to three (3) year old class with five (5) children was transitioning to after care at the end of the head start day. The educator took this class and another class upstairs to the aftercare educator and left for the day. Once the aftercare educator did a head count, she realized a child was missing since she had the class earlier in the day. The aftercare educator notified the cook who is also the parent of the child who was missing who went and got the child who was asleep on their mat. See Below for Detailed Information Observation:On May 3, 2021 Licensing Consultant Kirsten Turner verified an incident of a three (3) year old child being left alone in a classroom that occurred on Friday, April 30, 2021 as reported by the director who was not on site at the time. On April 30, 2021 around 2:30 pm the two and a half (2 ½) to three (3) year old class with five (5) children was transitioning to after care at the end of the head start day. The educator took this class and another class upstairs to the aftercare educator and left for the day. Once the aftercare educator did a head count, she realized a child was missing since she had the class earlier in the day. The aftercare educator notified the cook who is also the parent of the child who was missing who went and got the child who was asleep on their mat. The child had been unattended in the classroom for approximately five (5) to ten (10) minutes. 1240-04-01-.11 SUPERVISION
