During an unannounced visit for a complaint investigation, LC2 Betsy Hawthorne heard an adult shouting in a gruff voice, "Sit down!". LC proceeded to the location of the voice which was the older toddler room. Upon entering LC observed an educator with her back to the door leaning over two toddlers with a threatening attitude shouting "One! Two!" The co-educator in the room, was changing diapers and stated, "[Educator name} someone is here to see you." The educator turned and gasped and was visibly startled. LC conducted interviews with both educators regarding the complaint.

Additionally, an infant was not checked for breathing within 15 minutes during naptime as required 1240-04-03-.06 SUPERVISION Corrective Action: All staff were retrained in SIDS prevention and viewed the Before You Begin DVD again which overviews SIDS. Staff will also call out the infants name to ensure all infants are touched very 15 minutes as required
