See Below for Detailed Information Observation: During a complaint investigation it was confirmed that an onion was fed to a child but the child was told it was an apple. This was done as a joke to see the child’s reaction. Staff 1240-4-01.06 Corrective Action. This staff is not longer with the agency. Staff were retrained on appropriate behaviors. Observation: During an unannounced complaint investigation visit, Licensing consultant (LC) Brooke Ownby observed an educator in the one-year old room who was not directly supervising the children during mealtime. One child was playing, one child was walking around the room while drinking from a cup, and the remaining four children were eating and seated at the table. The educator was completing paperwork while standing away from the table. Her back was partially turned, so she was not able to supervise the children during mealtime. LC Ownby immediately got the Director to go into the classroom and correct the supervision issue and provided technical assistance on proper mealtime supervision. 1240.04.01.11 Supervision Corrective Action: During meal time, an educator will directly supervise the children while they are eating.

Observation: As part of the complaint investigation it was found that on May 24 two educators had an altercation on the playground in which the director stepped in between the two educators while there was yelling and name calling. This was inappropriate action and unsuitable behavior in front of the children. It resulted in children talking to their parents about the behavior and being afraid to come back to child care the next day. 1240-04-01-.06 STAFF Corrective Action: Staff have left the agency.
