On 10/3/25, LC Chastain and FS Pass made an unannounced visit for a complaint investigation. It was found that there were 3 children who's sign in and out sheets did not have dates. There were also 3 children who were not signed in for the day. On 10/3/25, LC Chastain and FS Pass conducted a complaint investigation. Ms. Lane was present with nine children, 5 children were under 3 years of age and 4 children were 4 years of age. Her assistant was absent from the home. Her substitute assistant was in another room in the house behind a closed door.
