Observation: Staff were observed seated in a chair on their cell phones and on the carpet while the toddlers were eating lunch. Staff was not focused on the children. 1240.04.03.06 Supervision Corrective Action: Staff have been retrained on mealtime supervision; additionally, the staff member responsible for the lapse as been terminated.
