Summary of Complaint, Self-Reported Incident, and Serving of a Safety Plan Visit Today's visit included monitoring the agency's compliance with Licensing Rules and Regulations, investigating a complaint, self-reported incident, and serving safety plans. The agency understands that the safety plan is in place until a lift letter is received from DHS Licensing. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. A supervision lapse occurred at the agency. Agency did not report an incident to TDHS on the same day as required. Agency did not report to Department of Children's Service's immediately as required. The agency did not provide documentation to the parent/guardian on the same day as an incident.
