Unannounced Complaint Investigation #PCS1004329/Safety Plan Served - The visit conducted today included monitoring the agency's compliance with Licensing Rules and Regulations. LC monitored the following areas – Supervision and High-Risk areas. This was a joint visit conducted with the DCS Investigator. Monitoring / Observations: DCS Investigator and LC conducted interviews with staff. DCS will continue the investigation. LC served the supervision safety plan. LC discussed the supervision log with the Director and provided the agency with a copy of the supervision log. LC discussed the requirements of the safety plans.

Summary of Monitoring Visit Complaint Investigation - Complaint # PCS 1002133 The visit conducted today included monitoring the agency's compliance with Licensing Rules and Regulations. LC monitored the following areas - Program, Language and Literacy Development- Behavior Management and Guidance; Supervision - sign-in and out sheets and Health & Safety - signs of illness; sick child policy. Other Monitoring/ Observations: LC observed all classrooms. LC started the investigation by interviewing all staff, reviewing sign-in and out sheets from January 2, 2023, thru this date, and reviewing the sick child policy. All staff reported that the yelling allegation was not valid. All staff reported that the sick policy is followed and that doctor's note is required per the policy. Sign-in and out sheets - Two staff reported that one person did not accurately record the arrival time correctly for two children and the time was changed to reflect the correct arrival time. All sign-in and out sheets reviewed appeared to be correct. Technical assistance was given to Director for staff to oversee when parents are signing in and out to ensure that correct times are being recorded for the children. The complaint was found to be invalid. No violations were noted. Two staff are required to complete five-year background checks soon. LC will send the notice when received by the background check unit.
