Summary of QRIS Monitoring Visit Today's visit included monitoring the agency's compliance with Licensing Rules and Regulations and Serving a Safety Plan. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. It was observed that 2 children, ages 3 years 1 month and 2 years 7 months were grouped with the Onederful Ones (13-30 months) group. The youngest child in the Onederful Ones was 1 year 1 month.

Complaint Investigation - Complaint # PCS 1002157 – Continuation of the complaint. DCS received a screened-out referral that is associated with the first referral/complaint. LC will investigate the allegations of the complaint. The visit conducted today included monitoring the agency's compliance with Licensing Rules and Regulations. LC monitored the following areas – Supervision – Playground; High Risks; Incident Reporting; and Program, Language and Literacy Development. Monitoring / Observations: LC observed the Pre-K classroom during outdoor play and all staff were appropriately supervising and interacting with the children during outdoor play. LC did not observe any areas of the playground where a child could hide or not be seen by staff. LC reviewed the playground supervision plan for the classroom and was in compliance. LC discussed the personal safety curriculum and staff had stopped teaching some areas for a few weeks due to behaviors among the children. Those behaviors were addressed and the teaching of the curriculum has resumed. LC discussed incident reporting, and the agency started documenting this week on an incident form any behaviors involving inappropriate behaviors or actions with the children. LC reviewed the forms. These reports are shared with the parent of the child involved. At this time, appropriate supervision was observed, and LC has no concerns at the agency. LC will continue the investigation with DCS. LC observed all other classrooms during the visit. LC will email the agency the Sexual Development and Behavior in Children handout. Reminder to complete updates on the emergency preparedness plan and email to LC before end of February 2023 if possible. No violations were received.

Summary of Monitoring Visit Complaint Investigation - Complaint # PCS 1002157 The visit conducted today included monitoring the agency's compliance with Licensing Rules and Regulations. LC monitored the following areas - Supervision; High Risk and Health & Safety. This was a joint visit conducted with DCS Investigator. Monitoring / Observations: DCS Investigator and LC conducted interviews with staff. LC and DCS Investigator observed the Pre-K classroom and restroom area in that classroom. The investigation has not identified any staff members or lack of supervision at the agency. Also, at this time DCS or DHS does not have any concerns at the agency. DCS will continue the investigation. LC observed all other classrooms during the visit. LC discussed with Director about ensuring foster grandparents are not performing caregiving duties and are having appropriate conservations with children. No violations were received.

Summary of Monitoring Visit Complaint Investigation - Complaint # PCS1002157 The visit conducted today included monitoring the agency's compliance with Licensing Rules and Regulations. LC monitored the following areas – High-risk areas and Health & Safety - restroom procedures at the agency were observed. Other Monitoring/ Observations: LC observed all classrooms. LC observed toileting in the preschool classroom. LC informed the agency that a complaint was received and DCS will be leading the investigation. Names of agency staff in the Preschool classroom were collected. Three children were not signed into the program. All three parents returned to the agency during the visit and signed their children into the program. LC suggested that the sign-in book be placed in the classroom during the morning arrivals to help ensure that all children are signed into the program. Also, educators need to review sign-in sheets to ensure that all children have been signed into the agency, and if not then parents will need to be contacted to return.

Corrective Action: Caregivers will receive training in mealtime supervision. Observations: A caregiver was performing other tasks and not seated or focused on a child who was eating during AM snack as required. 1240-04-03-.06 SUPERVISION. Corrective Action: Caregivers will receive training in mealtime supervision.

See Below for Detailed Information Observations: The agency did not maintain volunteer record information on site as required. 1240-04-03-.05 OWNERSHIP, ORGANIZATION, AND ADMINISTRATION. Corrective Action: The agency created a volunteer notebook to ensure that required documentation is kept on site. Observations: A caregiver in the 2.5 to 3 year old classroom was observed outside of the classroom leaving the other caregiver that was inside of the classroom with 10 children and out of ratio. The required ratio for this age group is 1:9. The violation was corrected on site when the caregiver returned to the classroom. 1240-04-03-.06 SUPERVISION. Corrective Action: Corrected on site.
