Observations: Four (4) children were not signed out of the program by a parent/guardian or authorized representative as required during the week of March 11-15, 2019. 1240-04-01-.11 SUPERVISION Corrective Action: Agency will ensure that parents/guardians are handed the sign in/out book at arrival and departure and agency staff will check to ensure that parent/guardians have signed children in and out of the program as required. Staff members will be retrained on enforcing the parent sign in and out procedures.
