Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint visit. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Inappropriate discipline was used in the toddler classroom.

See Below for Detailed Information Observation: Video footage showed teacher grabbed a child by the arm and then placed him on the floor. Another video was showed she grabbed another child by the arm. There was no sound on the video. 1240-04-01-.15 Program, Language and Literacy Development Corrective Action: The agency has terminated the employee.

Agency has received TA on this issue prior to citation. 1240-04-01-.16 Physical Facilities Corrective Action: Educator removed the blanket. LC will refer agency for CCR&R training. Observation: Licensing Consultant observed a 13 month old child sleeping in a crib with a blanket. 1240-04-01-.11 SUPERVISION Corrective Action: Educator removed the blanket. LC will refer agency for CCR&R training.

See Below for Detailed Information Observation: During a complaint investigation, Owner Ashley Arms stated that an educator was fired this week for hitting a child. The incident was not reported to DCS. 1240-04-01-.10 Duty to Report Child Abuse and Neglect Owner Ashley Arms was instructed to report the incident to DCS today and PE will send a referral to CCR&R for training.
