See Below for Detailed Information Observation: The areas where children play or are cared for shall be properly maintained, the grass needs to be trimmed and weeds need to be removed from under the equipment, and a overall clean up of the playgrounds. 1240-04-01-.16 Physical Facilities 9)c, 1,2 Plan of Corrective Action: Agency will have the playground cleaned up and weeded. Observation: When viewing the playground the climbers and swings did not have a protective fall zone surface as a shock absorbing, resilient material maintained with a minimum average depth of six (6) inches. 1240-04-01-.14 Equipment for Children 3)f Plan of Corrective Action: Agency will have wood mulch added to meet the six inch depth. 1240-04-01-.14 Equipment for Children 3)d Observation: PE observing the playground it was noted that the fall zone around swings did not extend twice the distance of the length of the swing chain. Plan of Corrective Action: The swings have been taken down, due to not enough room.
