Summary of the complaint investigation and serving safety plans Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and an investigation for 5 complaints and serving safety plans. During an unannounced visit for a complaint investigation on July 29, 2024, LC Bonnie Alvarez and FS Tiffany Felton verified with the agency that a 2-year-old child has been enrolled into a 3-5-year-old classroom except for 1 day he was put into a 2-year-old classroom. This child was put into this 3-5 year old classroom every day since the agency has been licensed for a Provisional License. During an unannounced visit for a complaint investigation on July 29, 2024, LC Bonnie Alvarez and FS Tiffany Felton observed an educator in the 3-5-year-old classroom leave her classroom and leave the building causing the children to be unsupervised during naptime. The educator left the classroom to take a child to the office who did not wish to nap. During an unannounced visit on July 29, 2024, to conduct a complaint investigation, LC Bonnie Alvarez and FS Tiffany Felton verified with staff that on Friday, July 26, 2024, 1-year-olds were brought to the 2-year-old classroom to combine 17 children for at least an hour on Friday afternoon. During an unannounced visit for a complaint investigation on July 29, 2024, LC Bonnie Alvarez and FS Tiffany Felton verified with staff that on Thursday, July 24, 2024, a 2-year-old child who was enrolled improperly in a 3-5-year-old classroom in the modular building left his classroom and the modular building on his own and was later found by an educator who was leaving for the day in the front lawn about 15 feet from the road. He was crying and very upset asking for his daddy. It is estimated that the child was outside unsupervised for a time of up to 30 minutes before an educator from another classroom noticed him. It was verified that the child's educator was asleep during this incident. During an unannounced visit for a complaint investigation on July 29, 2024, LC Bonnie Alvarez and FS Tiffany Felton verified with staff that on Thursday, July 24, 2024, the 1-year-old classroom was removed from their classroom and taken to the 2-year-old classroom and combined. This gave a total of 17 children for at least an hour that afternoon. The agency had enough staff that afternoon. The 2-year-old classroom only has a capacity of 14 children not 17.
