See Below for Detailed Information Observation: An 11 month old child was observed with a pacifier attached by a strap to her clothing which is prohibited. 1240-04-01-.13 Food and Food Service Corrective Action: The strap was immediately removed from the infant's clothing. The director informed all staff onsite that pacifiers should not be attached to clothing.

Licensing Consultant (LC) Melissa Odom has verified on 10/13/25 that an employee at the agency failed to complete the required Annual training Recognition & Reporting Child Abuse, Neglect, and Maltreatment by the September 30, 2025, deadline. Throughout the year, the LC provided technical assistance (TA) and multiple reminders to support the timely completion of these mandatory trainings. Despite these efforts, the training requirements were not fulfilled by the required timeline.
