The surfacing underneath the slide on the playground was not 6 inches. The riding toy's handle bar was removed by a child and then put back on. We asked the riding toy be removed. The staff removed the riding toy. There was potting soil in the two year olds bathroom on a low shelf. All keep out of reach items must be not accessible to children. We asked it to be removed and staff removed and locked up.

During an announced QRIS mointoring visit on 12/3/24 Licensing Counsulants (LC) Bonnie Alvarez and Melissa Odom observed a child in the bathroom in the gym unattended . Also the educator was observed leaving the play area in the gym and walking over to the eating area on the opposite side of the gym. During this time, her back was to the children and her focus was not on supervison of play time.

Observation: A two year old child was left alone in her classroom with Child Care Program Evaluators when her teachers and classmates went to the gym for lunch. Children should never be left unattended with persons who are not employed by the agency. 1240-04-01-.11 SUPERVISION Corrective Action: Staff will be re-trained on supervision requirements per policy. Staff will not leave any child enrolled in the agency unattended for any length of time.
