See Below for Detailed Information Observation: According to the video footage, a 2.5 year old was left in the classroom unsupervised. The supervision lapse lasted 16 minutes and when the teachers noticed the child was not with the class on the playground, one of the teachers went back to the classroom to find him. 1240-04-01-.11 (1) (a) Supervision Procedures Corrective Action: The teachers have been re-trained on roll call procedures to prevent further supervision lapses. Observation: A child was left in the classroom by himself. Supervision was not adequate. 1240-04-01-.11 (1) (c) Supervision Procedures Corrective Action: Re-training will occur for all staff to ensure supervision procedures are followed during transitions. Observation: Roll call was not completed before exiting the classroom. This caused the supervision lapse. 1240-04-01-.11 (1) (6) Playground Supervision Corrective Action: Staff will be re-trained in roll call procedures to ensure that no children are left behind during space to space transitions.
