See Below for Detailed Information Observation: Camera footage showed that a three year old was left on the playground without the knowledge of the staff. 1240-04-01-.11 (1) (c) 3. Supervision Corrective Action: All staff will receive training through CCR&R on supervision (both indoors and outdoors). The gate/fence will be made more secure so that children are not able to open the door by themselves. The camera footage showed that the child was left on the playground. After being left on the playground he opened the gate to the playground and walked to the building door. The teacher did not notice the child was left on the playground which caused the supervision lapse. The teacher was also not actively watching children. When she entered the building with the class she was seen on camera using a mobile device. 1240-01-.11 (1) (a) Supervision Procedures Corrective Action: All staff will receive training through CCR&R on supervision procedures. Observation: The camera footage showed that the educator pulled the child by the arm into the building. Potentially frightening and injurious discipline should never be used with the children. 1240-04-01-15 (5) (c) Behavior Management and Guidance Corrective Action: All staff will receive training on appropriate discipline methods. Observation: The camera footage showed that no roll call was done before the children left the playground with the educator. This caused one child to be left outside on the playground. 1240-04-01-.11 (3) 6. Playground Supervision Corrective Action: All staff will receive playground supervision training, active supervision training, and general supervision training through CCR&R.

The parent was notified but not in writing. 1240-04-01-.09 Incident Reporting Plan of Corrective Action: Will implement new incident forms and training with staff on procedures when informing a parent.

See Below for Detailed Information Observation: During the interview of staff, it was noted that some staff focus on unacceptable behavior with some of the children. Corrective Action: Agency states they plan to address this with staff members about when children are having issues to not solely to focus on the unacceptable behavior. Director will get work on getting some tools for the staff to have when working with these issues. CCR&R is also doing training with all staff on these matters. Director will be stopping in the classrooms more often. 1240-04-03-.09 Program Observation: During the interview of staff it was apparent that staff needs to have a better understanding of children’s attention spans and skills for their age. Corrective Action: All agency staff will receive training from Child Care Resource and Referral on classroom management. The Director will spend more time in each classroom. 1240-04-03-.09 Program
