Summary of QRIS Monitoring and Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, scoring of the QRIS, and complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Missing Evidence: - Proper handwashing procedures and policies not followed. (Gloves not worn for serving meals. Children did not wash hands after meals. Children did not wash hands after soiled diaper changes.) Missing Evidence: • Items labeled "keep out of the reach of children" are locked when not in use with the exception of hand soap. Bleach water or disinfectant/soap water must be inaccessible when not in use. (Handsanitizer and lotion inaccesible but not locked. Baby wipes accessible to children.) Missing Evidence: • Name-to-face roll call must be performed during all transitions. Missing Evidence: • Books about feelings and emotions must be present in the classroom and utilized as a resource to help children better understand their own feelings and emotions when applicable. (In the Toddler Classroom, books about feelings not utilized to help children.) Missig Evidence: • Educator modifies the learning and play environment to support appropriate behavior. (Schedule, routine, transitions, etc.) • Behavioral interventions shall be developmentally appropriate, with consideration given to the attention spans and skills of individual children. Missing Evidence: • Classroom rules are posted and reviewed with the children to teach and reinforce appropriate behavior. (In the Toddler Classroom, classroom rules are not posted.) Missing Evidence: • Parents given a daily record of their infant's activities such as feeding, diapering and napping. (Educator Cheryl fills out the children's daily sheets in advance, although the information may not be accurate.) Missing Evidence: • Family Advisory Board works with the director and educators to meet the needs of all children and parents. The board must have input into the program. Children did not wash their hands after lunch. Cook transports all food on a cart to each classroom. Plates with food are not covered and the Cook was not wearing gloves as she transported and delivered food to the classrooms.
