Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans The child care agency failed to report to DCS within the required timeframe and following required procedure. The agency did not fully cooperate with all agencies involved in the investigation of child abuse or neglect including preventing appropriate investigators from the Department of Human Services and the Department of Children's Services from conducting interviews with children and staff at the agency. Agency management failed to ensure that staff consistently provided appropriate supervision to children in their care resulting in a lapse in required oversight. Agency failed to report a serious supervision incident to parents upon pick up and failed to report to DHS as required. Evidence gathered including video surveillance and verbal admissions from multiple agency staff during LC Shrum's investigation has confirmed that a serious incident occurred on March 4th, 2026 involving a 2 1/2-year-old child walking out of the agency without the knowledge of the staff and was left unattended outside of the building for at least 15 minutes.
