Observation: On 7/10/19, PE observed a 10 month old in a pack-n-play with 4 soft toys and a wearing a bib. 1240-04-01-.11 SUPERVISION Corrective Action: The educator immediately removed the bib and soft toys from the crib. The director held a retraining for her infant staff on 7/21/2019.
