Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Agency will report incidents that happen outside of the normal day and make sure parents sign incident reports. Observation: The agency failed to report to DHS or DCS that a child had exited out of the building. 1240-04-01-.10 DUTY TO REPORT CHILD ABUSE AND NEGLECT Corrective Action: Director states that she will report all major incidents to DHS and DCS. Agency will be referred to CCRR for training on proper ways to report. Observation: During last summer, a child exited the gym doors to the outside of the agency without educators being aware the child was gone until time had passed. 1240-04-01-.11 SUPERVISION Corrective Action: The agency has put up metal sliding fences to block the doors when there is children present in the gym. Agency also put alarms on the gyms doors and positioned an educator at each door leading to the outside of the agency. The agency also has cameras in the gym. Agency will be referred to CCRR for supervision. Observation: Agency failed to notify the Department that an employee had been named in a pending warrant, indictment, presentment, or petition in a timely fashion so that criminal background could have the employee’s fingerprints ran as soon as possible. 1240-04-01-.07 Criminal Background Check and State Registry/Records Review Procedures Corrective Action: The employee has been excluded from the agency until further notice. The director will report any arrests within 24 hours to the Department in the future.

Children were observed eating dropped food off of the floor and leaving the table to play with food in their hands and mouths.
