Director self reported that a child was left in the classroom during a fire drill on 2/25/2021. 1240-04-01-11 Supervision POCA-As a staff we met on 3-1-21 and reviewed the importance of tracking children during their transitions in and out of classrooms. We as a staff, rolled played this procedure to ensure everyone knows the steps to follow-Face to Name in and out of space, face to name into the new space, face to name out of that space and face to name arriving back into the original space. This meeting was also attended by out board chair who also spoke to the staff about how serious this was. We will seek training from CCR&R on the topic of safety and supervision. The two staff involved in this incident have been placed on 6 months probation with specific trainings to complete via the pro -Solutions web site. These trainings relate to the topic of safety and supervision. We will review the tracking and supervision polices on a regular basis with all staff.

Observation: One staff has been reported to DHS/DCS. She was said to have popped a child on her leg upper thigh area. 1240-04-03-.09 Program (5) (c) (e) Corrective Action: Director reported ...After the child reported this to their teacher, the other teacher was immediately sent home and told not to come on the next day. She vehemently denied hitting the child. Admin reported to parent that day and told her we would investigate. We also reported to DCS and to our PE. DCS gave us a quick response and said the event did not meet their definition of child abuse. Our PE instructed us to make a report to the DHS complaint line. The teacher was terminated for inappropriate discipline since the hit could not be proven.

Observation: Management has failed to report, acknowledge and or look into complaints (venting) from staff as well as documenting or failing to properly train staff in reporting abuse.However, in this particular incident Management did take the correct steps in reporting to DCS and then DHS. 1240-04-03.06 Supervision (1) (a) 1. Corrective Action: Director stated...any time a teacher shared a concern, it was followed up on. The teacher making the concern would not have been informed of the action taken. With and incident as serious as potentially hitting a child, as in this case, a report was made to the parents, DCS and DHS . We investigated the allegation and determined that while we could not prove the child was hit, we did need to terminate the employee. This was done before DHS investigated. the staff are trained on child abuse reporting procedures every six months. We last trained on this at staff meeting on 11.14.17 when we strengthened our policy. Then again on 1.21.18 the staff were giving a copy of the church's policy which mirrors ours, and asked to sign it. We welcome and appreciate the suggestions of our PE. We will review the cell phone policy-two employees that were observed by PE using their phones at an inappropriate time received counsels after her visit. Review child abuse prevention and reporting every six months as required.
