Observation: Program evaluator observed video footage from 3/01/21 of a 5-year-old who was left unattended in a classroom for approximately 2 minutes. 1240-04-01-.11 SUPERVISION Corrective Action: The caregiver will ensure children walk in front of or beside her when moving locations and count at each destination. The director will change policy and allow parents to take their children directly to the classroom.

OBSERVATION: Educator was getting diapering area ready for a child when she notice a child had picked up a chair and was swinging the chair. She went over to the child and took the chair away and redirected him. Another child slipped through the changing room and went into the 2 year old classroom. The child then went in the hallway where an Educator was waiting for the bathroom. The Educator immediately took him to his classroom. This was self-reported. 1240-04-01-11 Supervision Corrective Action: Children will be supervised at all times.

OBSERVATION: The teacher stated this little boy hugged her and then he came back 3-4 more times and she made the comment that he was gonna grow up to be a rapist. 1240-04-01-.15 Program Language and Literacy Development. (5) Behavior management and Guidance (h) The educator shall address each incident of unacceptable behavior by using methods of positive guidance and discipline to help the child manage his/her behavior.
