During a complaint investigation it was confirmed that a child was on the playground eating without an educator directly supervising; licensing rules state that an adult must be within director sight and sound the child while they are eating. During a complaint investigation it was confirmed that the maximum capacity of children for the age grouping were on the playground at one time, resulting in potential safety hazards and risks. Corrective Action: Upon the child’s enrollment, checklist items will be required. After two weeks, the child will not be able to return. 1204-04-03-07 Staff Observation: There was no proof/documentation that a staff member received trainings for the following: shaken baby syndrome/abuse head trauma and a pre-service training recognized by the Department as required. Corrective Action: The agency will create a checklist to ensure all trainings have been completed when new hires come to the agency on the first day. Observation: The agency received nine immunization deficiencies February 27, 2018.
