Summary of Monitoring Visit The visit conducted today included investigating a complaint/incident that occured on 05/10/2023 including a student being left on the playground. LC Holden interviewed the site director as well as reviewed surviellance footage to confirm the incident. Technical Assistance Provided: 1. Ensure that staff are using name to head count to keep up with the number of students they have and that they do it before and after every transition to and from any room/area. It also included a review of the safety plan. LC Holden will assist the Agency with any requests or assistance needed to assure completion of all tasks from the safety plan. No concerns were observed or reported during the visit. LC Holden also investigated a complaint made about a recent staff/child interactions. Summary of Monitoring Visit The visit conducted today included investigating a complaint made on 02/09/2023 concerning a staff educator falling asleep in the classroom during naptime as well as another complaint of a staff educator patting a child excessively in attempt to get them to sleep. LC Holden, LC Barnes, and LC Ratliff interviewed the site director as well as the Chief operator. The alleged staff was indeed sleeping after reviewing a video but because there were two staff and one was awake, no violation was cited. The video of the alleged incident concerning a staff patting a child too hard to sleep did not show any substantial evidence of harm and only depicted audio of a child crying while clearly being patted on. Because there is no evidence of any type of force, abuse, or inappropriate interaction, no violation were cited. Technical Assistance Provided: 1. Ensure staff are aware of proper napping procedures and restful alternatives to sleeping. 2. Ensure staff are being mindful of how they interact with children and how it may appear to the children as well as other staff. No Violations were received

Summary of Monitoring Visit The visit conducted today included investigating a complaint made on 01/11/2023 concerning a parent reporting that their child was inappropriately touched by two other students. The Director informed the LC of the incident and that she would be reporting it to DCS as well. The LC arrived at St. Luke's Community House on 01/18/2023 to complete an investigation. The director was reportedly out sick, however, the chief operator was present for the LC to consult with. Technical Assistance Provided: 1.
