See Below for Detailed Information Observation: A 5 year-old child left the agency and was gone approx. 30 minutes. The agency was unaware until the child was returned by law enforcement to the agency. 1240-04-01-.11 SUPERVISION corrective action: The agency is working on an updated supervision plan to prevent this from happening in the future. observation:The incident of the 5 year old child leaving the agency unsupervised and being return by law enforcement approx.. 30 minutes later was not reported to the DHS. An incident report was not completed and provided to the parent. 1240-04-01-.09 INCIDENT REPORTING corrective action: The agency advised they will complete incident reports in the future in the event an incident occurs. Observation:The sign in/out sheet for the agency only notes the child's name, date, a space for the parent to sign, and space for the time out to be noted. It does not have a space for the authorized individual to print their name. 1240-04-01-.11 SUPERVISION corrective action: The agency is working on revising the sign in/out sheet to meet compliance. observation: The children were on the playground. When returning inside, the educator did not complete a name to face roll call and returned in the building in front of the children without assuring all the children returned in the building. 1240-04-01-.11 SUPERVISION corrective action: The agency will update the playground supervision plan. Observation:There were three staff present on 9.27.2021 and staff present on 9.29.2021 that have not completed/cleared the fingerprinting through TDHS. 1240-04-01-.07 Criminal Background Corrective action: Agency will send staff to complete fingerprinting and will not be at the agency until complete and cleared. .
