Summary of QRIS Monitoring Visit and Complaint The visit conducted today included monitoring of the agency's Compliance with Licensing Rules and Regulations and scoring of the QRIS and to investigate a Complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Observed: Program evaluator observed there were children 12 children mixed together ages 14mo-4yrs with one teacher. After the toddler teacher arrived, the children were separated into the correct classrooms. 1240-04-01-22: Child Care Center Specific Requirements Corrective Action: Educator immediately separated the children to be in compliance. Observation: PE observed the teacher placing an infant in a highchair with dry cheerios. Educator walked away to the kitchen area to pour a cup of coffee and then came to the table to sit (away from the highchair). PE advised Educator the child had to have direct supervision during meal time. 1240-04-01-11 Supervision Corrective Action: The action to prevent this from happening again is to go over the section of rules for the mealtime supervision and to make sure the rules are understood, as well as, taken action on.
