It was raining outside and the children did not go outside but LC did not observe any classroom do ing indoor physical activities. LC gave Director a reminder to make sure each educator is doing this indoor physical play if they can't go outside due to weather.

Director did not notify the Department the same day that a complaint from a parent came in about an educator and the educator was fired. Director did not notify DCS of this complaint which would be considered suspected verbal abuse. Director did not report in the DHS portal an incident where a phsycial altercation occurred in the infant room between two employees and police were called. This phsycial altercation was also not reported to DCS.

Food was served to childen prior to edcuator being seated and her back was turned while plating the food. Also educator got up with children to assist in washing their hands and left one child at the table with food and her back was turned to him. Educator had children sit down to read them a book but left all food on the table and some children did not sit and were running around the table and could have eaten the food.

LC Weston observed educator with their back to a child still eating. Educator got up to help other toddlers wash their hands while one toddler was still seated and eating. Her back was turned for several minutes. Food was also left out after mealtime and not cleaned up.
