Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint Investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. During a complaint observation I was informed by the provider that when she was revieweing video an educator steeped away from the table while children were eating and left the classroom. During an unannounced visit, LC Kelley conducted interviews and it was determined that the director observed on video and aware of an educator walking out of the classroom. During a complaint investigation it was discovered that information in the complaint stating children being "hit with broom" " Hit up against the wall" is valid During an unannounced visit, LC Kelley conducted interviews and it was determined that an educator handled children in an inappropriate manner. During a complaint investigation it was discovered that an incident occurred with a lapse of supervision and an incident report was not written nor were parents informed by the end of the day. Provider observed this incident on video during an observation.

See Below for Detailed Information While reviewing the video on 12/13/18 @9:30am-10:15am, the teachers took the 2 year old class (11 children) outside to play in the parking lot of the facility. The parking lot is not secure or fenced and open to vehicle traffic. 1240-04-01-16 (9)(b) corrective action plan: Continued training through CCR&R and further classroom monitoring CCPE’s observed a child in the 2-3year old room that was not enrolled(thechild of an current employee) in the center or assigned to the classroom. 1240-04-03-05(e) Corrective action plan: further training through CCR&R and classroom monitoring The teacher did not follow safe sleep practice by not checking three babies every 15 minutes that were in their cribs. One child was brought in at 8:18am and laid in the crib by parent. A teacher did not check or observe the child for 45 minutes. Two children were sleep in their crib and was never checked during the time of observing the video.
