Summary of complaint investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. It was discovered during a complaint investigation, that children have been shamed , humiliated, frightened, verbally, and physcially abused, this includes pinching, grabbing, ear pulling and bending of the toes. Targeted children were between the ages of 12-30mths. It was also discovered that a 3 year old male suffered from corporal punishment. It was discovered during complaint investigation, that suspected child abuse and neglect was reported to director and assistant director by several educators. Educators did not report to DCS nor did the adminstrative staff. It was discovered during a complaint investigation, diapers were not checked throughout the day. Children were not changed immediately when solid or wet. During complaint investigation it was discovered that the director and assistant director had knowledge of an incident that took place on multiple occasions that may have put toddlers in a harmful situation. Directoror assistant director did not report this to DCS or DHS. During complaint investigation it was discovered that food was being withheld from children due to behavior at the table. Children were not given certain items of food that were messy. Children were not offered milk on certain occasions; this would be if the children did not eat their food or eat food that they did not like. Food was thrown away if children did not comply to rules at table.
