See Below for Detailed Information Observation: There was not proof/documentation of an updated health exam for one staff member and there was no no health exam for another staff member as required. 1240-04-03-05 Ownership, Organization, and Administration Observation: There was no proof/documentation that two staff members received shaken baby syndrome/abusive head trauma as required. 1240-04-03-07 Staff Observation: The electrical cord to the heater was accessible to the children in the Infant II class. It is required that cords not be accessible to the children. Violation was corrected when the heater was removed. 1240-04-03-08 Equipment Observation: Two staff members were observed with twenty-four children ranging in ages from twenty-seven months old to five years old.
