Summary of Monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations. It also included a review of the QRIS. Technical Assistance Provided: 1. Name to Face when transitioning children 2. Add additional step of a visual sweep of the room and a second name to face on the playground to assure all childdren are accounted for. Violations were received and include: 1. Supervision 1240-04-01-11 Correct Action Plan (CAP) completed. Violation details can be observed below. During transition from the classroom to the playground, after using name to face, as the teacher was reaching for an item needed by a child on the shelf, a child ran back in the classroom. The door closed, momentarily separating the child from the teacher. The teacher immediately opened the door retreiving the child, and notified the director of the incident.

Observation: During a complaint investigation, LC noted that management failed to maintain a system that enables all children to maintain a level of supervision appropriate to their age. . 1240-04-01-.11 SUPERVISION Corrective Action: Technical assistance was given. Agency, including management will receive training in Supervision.
