Licensing Consultant Kelley was informed that a child was left unsupervised through the DHS e-Licnesing Portal. During an unannounced visit, LC Kelley spoke with educators and it was confirmed that a child was indeed left unsupervised in the stairwell during the transition from the playground.

1) Reports of Incidents, Illnesses, Accidents, Injuries, and Fatalities. (d) Incidents, accidents, injuries, and signs of serious illness to children shall be documented immediately with the following information: 1. Child's name and date of birth; 2. (2) Supervision Procedures. (a) To ensure the health and safety of all children enrolled, the management of the child care agency shall maintain a system that enables all children to receive a level of supervision that is appropriate to their age and their developmental, physical and mental status. The child care agency staff shall be within sight and sound of the children in their care at all times, be aware of their activities, and be able to intervene appropriately c) If any child is left unattended at any time, the child care agency is subject to legal enforcement action.
