During an unannounced visit to investigate a serious injury incident, LC observed a 5 year old child brought inside by an educator to use the restroom. The child went into the class then into the restroom without an educator present in the classroom. The educator sent the child into the classroom alone. The child was left unattended in the restroom and classroom for approximately 2 minutes until educator went into restroom area with child.
