Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and scoring of the QRIS. Technical Assistance Provided: 1. Always Name to Face Children when transitioning to and from the playground. 2. Have maintenance secure the spikes for the playground trim around the mulch If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. 1240-04-01-.11 (2)(a); 1240-04-01-.11 (2)(3); 1240-04-01-.11 (4)(b)(6) Student left unattended on Playground. Child left unattended on playground. A child was left unattended on playground.
