Documentation, signed by the examining licensed physician, licensed psychologist, licensed clinician, Nurse Practitioner or Physician’s Assistant, verifying that the staff person is capable of safely and appropriately providing care for children in a group setting. The documentation shall be on file within ten (10) calendar days of employment or starting to work. An updated statement of each staff member’s physical health shall be obtained every third (3rd) year, or more often if deemed necessary by the Department; OBSERVATION: Staff files were reviewed and 6 of 8 were found to not contain required statement of physical health.
