Summary of Monitoring Visit and Complaint Investigation The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, a review of the QRIS, and a complaint investigation. Technical Assistance Provided: 1. LC and director discussed the emergency plan template and that every space must have something in it. The director has the template from last year but will update it, add page 10, and sign and date it. 2. LC and director discussed that incident/accident reports should be completed any time there is an incident or accident, no matter how minor. Parents should receive a signed copy of the report and the report should be kept in the child's file. 3. LC informed the director that the annual report will open in the provider portal at the end of March. Violations were received and include: 1. Incident Reporting in the Toddler Room Correct Action Plan (CAP) completed. Violation details can be observed below. Incident reports were not completed as needed for children in the Toddler Room.

Observation: A biting incident took place in the Toddler classroom. When the incident occurred, one educator was changing diapers and the other educator was talking to someone in the neighboring classroom. The educators did not observe the biting incident. Also, a sensory bottle in the Toddler classroom was opened, the contents spilled onto the floor, and a child got some in her mouth. The children were not being supervised closely enough to prevent the bottle from being opened. 1240-04-01-.11 Supervision Corrective Action: The staff were "written up" and informed to supervise children at all times. The director emphasized that talking between classrooms should not happen. CCR&R visited the agency to model behavior and offer suggestions on supervision.

Observation: Video footage of the Infant classroom was reviewed and showed that a child tipped the bottle warmer over on herself on two separate days. The video surveillance footage also showed that the child was left on the changing table unattended. While unattended, the child almost rolled off of the changing table. 1240-04-01-.11 SUPERVISION Corrective Action: The above educator has been terminated for not following supervision and safety measures. All other educators/floaters in the infant room have been instructed on proper supervision of children while in highchairs or changing tables. They must be strapped in and not left unattended. Safety and supervision training will be required by the educator also. We no longer have a bottle warmer in use and if any bottles do need warmed up, the educator will call the director to take the bottle to the kitchen to run under the hot water to warm it up. We will also allow parents to bring the powder and just add water if the child is not taking breast milk. This way, bottles will be warm with the water added. Educators have been instructed that they must maintain supervision at all times in the classroom and maintain proper health and safety measures of all of the children in their care. The director will also be doing extra safety checks in this classroom to make sure it is being done.

Observation: During interviews with the staff, the educators confirmed that they applied a topical ointment to a child’s skin when trying to provide relief to the child. The topical ointment did not belong to the child. 1240-04-01-.12 HEALTH AND SAFETY Corrective Action: Educators have been instructed that all diaper creams must have the child's name on it and permission slip must be signed by the child's parent and state what cream it is on the permission slip. No diaper creams may be used on other another child. All medications must also have the child's name and instructions on them and be locked in the office in the lock box. Medication forms must be filled out by the parents and signed and have the exact information on the medication bottle on the form. Only center director or assistant director may give medication. Medication will be sent home daily and parent will sign it back out.
