Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and a complaint Investigation. If any violations were received, details can be observed in the compliance details section of the visit, along with corresponding corrective action plans. On Wednesday, 05/13/26 a child in the Transition 1 room climbed over shelving and exited the classroom. The child then entered the staff breakroom, where another educator was present who called out to classroom-Transition 1 and let them know he was there and returned him to the educator in charge of the classroom, who had been assisting with handwashing with her back to the classroom. On Thursday, 05/14/26 a child in the Transition 1 room opened the door that is the direct exit outside and the educator heard the door opening and called out to the child who then ran around the front of the building. The educator called for help and ran out of the classroom and retrieved the child within a minute. Summary of [Complaint] Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and [reason for visit—such as complaint]. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. LC overheard educator yelling orders at child(ren). Educator did not use enrichment language and did not address the incident of unacceptable behavior by using methods of positive guidance and discipline to help the child(ren) manage his/her behavior. In the 6 wk - 12 month old classroom the TA left the room to gather some items making the infant room out of ratio. The ratio is 1:4. When TA left the room the infant room was 1:5. This was verified by LC watching video. This agency self reported this incident.
