We discussed the sign in/out sheets again and making sure they know who is at the agency. LC previously suggested highlighting the name to make sure they showed that they are aware that the child is not signed in and to inform the 2. We discussed that the sunscreen and potting soil needs to be kept out of reach. 3. We discussed reporting abuse/neglect. The agency did not have completed sign in and sign out sheets The agency was informed that a child had bruises and did not report it to DCS

Mold in the classrooms suggested child safety locks on the cabinets. 2. Pre-play inspections 3. Keep out of reach items being locked away 4. Supervision of SA children 5. Placement and securing equipment, inside and outside 6. Keeping children's files when they leave the agency. An educator's personal belongings were placed outside of the Pre-K classroom and accessible to the children. There was air freshener on top of a tall cabinet in one bathroom and disinfecting spray in a tall cabinet in the Pre-K room. They did not complete a pre-play inspection of the playground.

Summary of Complaint Investigation Visit. The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint . Technical Assistance Provided: 1. LC discussed that a child can not have a sippy cup or bottle just sitting around on the floor or walking around the room. 2. LC informed the Infant and Toddler room educators that the children can't have pacifier clips. 3. The educator in the Toddler room was letting the children watch a movie on her cell phone and LC explained that children under two (2) years old are not allowed to watch TV. 4. LC discussed the ratio and supervision issues with the educator. 5. LC discussed all of the violations with the educator. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Children were observed with a pacifier clipped to their clothing. There was a lack of supervision at the agency. Some of the classes were found out of compliance with the adult:child ratio. Children were observed with a sippy cup and bottle without supervision by an educator. Also, a child was observed walking around the room eating food with the Pre-K and SA children.

Observation: Upon entering the building (3 year old classroom) PE observed educator yelling at a child laying down at nap.

See Below for Detailed Information Observation: A one year old child was seated in a high chair eating cheerios. The same educator had her back turned to the child repeatedly and even left the room. Due to serious risk of choking, all children are required to have close direct supervision while they eat. 1240-04-01-..11 SUPERVISION Corrective Action: Program Evaluator (PE) told the educator that the child eating needed to be supervised. PE went over mealtime supervision rules with the educator and provided technical assistance. All staff will get training on Appropriate Supervision and Mealtime Supervision. Observation: One classroom had a 27 month old, a 4 year old, and a school age child with one educator. The correct adult:child ratio for a combined classroom can be 30 months to 12 years, but this classroom was out of compliance with adult:child ratios and grouping rules. 1240-04-01-.22 SPECIFIC REQUIREMENTS FOR CHILD CARE CENTERS Corrective Action: PE went over adult:child ratios and grouping rules with the educator and provided technical assistance. A plan of corrective action was developed. All staff will get training and the agency may face further consequences.. Observation: The same educator walked out of the classroom, leaving 2 children (an infant and a 1 year old) alone and unsupervised. Children need adult supervision at all times. 1240-04-01-.11 SUPERVISION Corrective Action: Program Evaluator (PE) told the educator children need to be supervised at all times and had her come back into the classroom. PE discussed the need for supervision and provided technical assistance. All staff will get training on Appropriate Supervision. Observation: A two (2) month old infant was asleep in a bouncy seat with a blanket. To help in the prevention of suffocation and Sudden Infant Death Syndrome (SIDS), infants are only to sleep in cribs or pac'n'plays. Further, when sleeping they are not have any soft bedding, such as blankets. 1240-04-01-.11 SUPERVISION Corrective Action: Program Evaluator (PE) told the educator to remove the sleeping infant from the bouncy seat. PE went over SIDS rules with the educator and provided technical assistance. All staff will get training on Safe Sleep Practices.

See Below for Detailed Information Observation: During the 4-3-19 assessment, staff and infants hands were not consistently washed when doing bottle feedings. Staff and infants hands are supposed to be washed before and after bottle feedings, to help prevent the spread of germs. 1240-04-01-.13 FOOD Corrective Action: A plan of corrective action was done with director/owner Nicki McCoy. PE Dufresne gave her technical assistance regarding proper procedures for bottle feeding. Nicki McCoy will review the food safety procedures with ALL her staff, since it could have been a floater that made mistakes, not necessarily the regular infant educators in that classroom.

While it is acceptable to combine children of these ages, the maximum group size is 10 children.
